阿伟（吴镇宇 饰）是社团中负责清理凶杀现场的“纽扣人”，昔日阿伟在担任警方与社团双料卧底之际，同伴“医生”（戴立忍 饰）恐怕自己的卧底身份败露，将阿伟重伤导致其失忆，自此，阿伟与医生搭伙，处理现场并将取自尸体的器官贩卖，支撑二人的，是日后定居小岛的梦想。不久，医生因卷入社团高层被杀事件导致自己被处死。失去了唯一同伴的阿伟更加消沉，然而这份需要绝对冷酷的工作还得继续……阿伟在社团的安排下，收徒（黄阅 饰）继续纽扣人生涯，和阿伟情人（关颖 饰）亦保持关系的徒弟心地颇为柔软，日日在血腥中摸爬的阿伟亦愈发感到厌倦，社团中部分人凶残的手法让他们越来越濒临心理的临界点……
