20世纪60年代，香港监狱乌烟瘴气，警匪一家，腐败横生。曾经专门为江湖兄弟打官司的律师陈兆康（吴镇宇 饰），在帮助资深小混混红孩儿（梁焯满 饰）打官司的过程中突发意外，导致出手伤了警察，结果两人双双锒铛入狱。典狱长（徐锦江 饰）和狱头食人鲳（龙方 饰）只手遮天，横行霸道，经常为了一己私欲对犯人大打出手。陈虽然看不惯他们的所作所为，却无能为力。而牢房内鱼龙混杂，各方势力云集，有人想对陈律师动手，有人则要一心维护，纷争一触即发。原本一身正气的陈律师显得格格不入，狱警和犯人又沆瀣一气对他进行整治殴打。 想要独善其身，万般艰难……
