明朝天启年间，左都御史杨涟因得罪魏忠贤含冤入狱，魏忠贤授意东厂及锦衣卫秘密将杨家灭门，徐长卿受王冬青所托秘密救援杨涟孙女杨紫萱，长卿舍身护送紫萱从应山府逃到龙泉村同仁医馆投靠刘谦，刘谦感念恩师杨涟恩情，将紫萱收作关门弟子细心照顾并传授其医学知识，此后几个人将医馆生意打理的井井有条，没想到却引来同行郎中吴南星的嫉妒，最后惊动魏忠贤党羽，魏忠贤得知杨紫萱踪迹派出多批杀手奔医馆而来，只为将杨家孤女紫萱和账本（杨涟所留魏忠贤通敌）一同斩草除根……江湖风云又起。 在韩佩兰徐长卿等人保护下刘谦紫萱准备开始转移，不料突遇梅李庄传来瘟疫，刘谦不惧生死多次救护军民百姓。最后，在刘谦和诚王和王冬青等人的鼎力帮助下，瘟疫得到控制，刘谦也赢来“侠医”称号，大太监魏忠贤终被扳倒，杨涟被崇祯皇帝平反并追封“忠烈”。
