1978年改革开放初期，百业待兴。安徽凤阳人因连年灾害穷困潦倒，苦不堪言，不得不逃到外省乞讨为生。是年，陈开元（李心敏 饰）调任凤阳县县委书记，他审时度势，顶住革委会高主任等保守势力的政治压力，深入基层群众了解当前各地的现状和疾苦。 小岗村生产队长严家昌暗中引导村民“分田到组”，调动农民的生产积极性。小岗村的做法得到陈开元的支持和鼓励，不顾病体帮助严开拓一条改革自救之路。小岗村十八户村民冒着违反原则的危险在大包干协议书上按下了手印……
