20世纪30年代，日本帝国主义贪婪的魔爪伸向古老的中华大地，它扶植早已逊位的溥仪为傀儡，在满洲里建立了伪满帝国，企图达到分裂并霸占中国的野心。在这乱世之秋，外科医生刘栋梁（辛柏青 饰）学成回国，在哈尔滨市立医院担任外科副主任。某天，日本宪兵将抗联女政委赵一曼押至医院，指名要求刘为其治疗。刘为赵一曼坚忍高尚的精神所感动，与此同时更懵懵懂懂地卷入营救赵一曼的行动之中。在此之后，他被警察逮捕，却由此和倾慕自己的麻醉师娜拉（苗圃 饰）结为夫妇。纳拉的父亲苏春来是死忠于伪满政府的顽固人士，他对新兴的革命政党怀有深深的误解。刘和娜拉伸出他的阴影之下无比痛苦。 偶然机缘，刘成为了一名地下党员，他由此走上了一条充满艰险的坎坷之途……
