Kenzo Tenma, a Japanese who works at Eisler Memorial Hospital in West Germany, is known as a "genius doctor". His surgical skills are superb and he is humble. He is envied by his colleagues and respected by doctors and patients. In terms of career, Tianma has a bright future and is favored by the dean; in terms of love, the dean's daughter Eva falls in love with him and they are like glue. However, everything seems to be going well for Tianma, but he gradually feels uneasy conscience. He witnesses with his own eyes that the hospital ignores ordinary people who are critically ill in order to save important people. Tianma suffers from the accusations and cries of the family members of the deceased. When this dilemma came again, Tianma resolutely chose to rescue a boy who was shot in the head. He sacrificed his career and love, but in exchange for the resurrection of a terrible monster. In the following years, Pegasus and the monster John started a battle of wits and courage, and a dusty history was slowly revealed... This film is adapted from the original work of the same name by the Japanese cartoonist Naoki Urasawa.
